Parents! please note... these exact questions have been on CDC surveys that have been given every 2 years in states across the nation to students in high school since 1991 and in middle schools since 1995 . Go to the CDC website www.cdc.gov and search on Youth Risk Behavior Surveillance System (YRBSS) for detailed info including which states have participated in which years (Maryland has only participated since 2005 except for a small participation in 1991.) The CDC website includes copies of all of the surveys given going back to 1991 for HS and 1995 for MS. The questions mentioned in the article above are on the 1991 and 1995 surveys, and I assume in every year since.

    The CDC's Youth Risk Behavior Surveillance System (YRBSS) monitors six types of health-risk behaviors that contribute to the leading causes of death and disability among youth and adults. To learn more see:

    The questions, both style and substance, are disturbing. I haven't shown them to a parent yet who hasn't become visibly upset. The current 2014 High School Survey is 99 questions. Apparently the CDC picks the exact schools to be given the survey in each county and directs MSDE (Maryland State Dept of Ed) to give the survey. MSDE hires a contractor to help. Notifications to parents (a Parent Information Sheet and a Parent Opt Out Form) are papers sent home for the classes that are going to take it. (notification is not electronic (email) per MSDE directions.)

    Does anyone know what Federal Law this CDC survey / access to HS and MS students across the nation falls under?
